Oops, shoulda read the thread...
By the way, I don't mean to be nasty but am givin' you some advice. I don't believe this is a very good research method for the subject. On understanding feelings you can't use pure statistics, however much you wish to believe that you may get qualitative data from this survey.
Try triangulation, case studies, that kinda thing.

It's just for a school project on teens and their self image. If anyone would like to coment further on their thoughts about the subject then by all means do, Just trying to use what little resources i have.
P.S. are you majoring in psychology? just wondering.

I've done psychology and sociology and research was my best area... The problem with your tests partially are because they're ethnocentric (biased towards your culture), we have a different schooling system to yours.
Personally I think that a survey wouldn't really do the topic justice, but I guess seeing as it's for school it doesn't matter so much as whether a large company were doing it.

The ethnocentricity of this research would result in biased results towards the western world. But then body image in some cultures is a meer thought. In your research/assignment you would need to state this, hey it'd even get you so extra marks. :)

ethnocentricity. sorry, you spelt it wrong. The first line makes sense whereas the rest of your statement does not, your assumptions that he needs to put in the differences between the cultures is preposterous. Each different culture will look at it in a different angle i:e Canada and America will view it differently as will Scotland will view it differently to England and so on and so forth.

Are you doing a piece of research too? qualitative data is ok, but it has it's limitations. If you want to get vast amount of opinions i'd say qauntatative is far the easiest and quickest. Though perhaps to get triangulation there should be a random selection process followed by interviews in more depth.
