I know that for my gp you have to live within 3 miles of the practise

within a practise, just make another appointment. Otherwise you have to register with another practise and most NHS surgeries will have a sort of catchment, like Mel said.
Then you need to fill in that little bit at the bottom of your NHS card and get your new GP to sign it.
That's just what I know, good ole NHS Direct (0845 46 47) will probably have more info.

Think you have to have a pretty good reason though, unless you move house. Not sure they're happy about you changing just randomly.

It doesn't matter why - you just find a GP practice that is accepting new patients and register.
NHS Direct won't be able to tell you which GPs are accepting new patients, and they wanted to know my name, address and phone number when all I wanted was details of the local GPs. Nosey parkers.
Try the website instead NHS Direct (http://www.nhsdirect.nhs.uk/localisation/)
